If necessary, please seek medical clearance to attend exercise/fitness/pilates classes. You understand that whilst every care will be taken to give safe instruction, you accept full responsibility and consider yourself fit to exercise. You have answered all the questions below fully and all medical and health considerations are noted. You will keep the instructor updated if any of the below information changes.
